inserting logo

I am unfamiliar with HTML and although I see the language that I need to insert (for my logo) I do not know WHERE to insert this on the HTML invoice template.

Second problem. When I first set up a Cashboard invoice, I chose the European date format dd.mm.yyyy
When I tried to change it back to mm.dd.yyyy and start a new invoice it doesn't retain the change.
